   I can't continue playing the game before I chose one of these options for my helmet.

In general:

 

1) take purple (epic) before blue (rare) before common

2) at equal rarities, take combat refinements before skill refinements

 

I would make an exception for first strike. I consider that refinement completely useless, and never take it at any rarity.

There is a level below completely useless. And that is "actively detrimental".

 

You might be able to make that case for first strike. I suspect but have never proven that opponents in fighting missions get refinements based upon the number you have. So in that case, wearing a bad refinement also makes your opponent stronger.
