Has anyone ever experienced a bug where the exclamation point on the yellow notebook at the top of the screen wouldn't go away after completing an achievement? If so how did you fix it?

Are you sure you don't have an achievement hidden, via the hiding options?
Just checking 
I have my energy missions hidden, so I don't grab energy rewards when not needed.

Usually these problems go away after you restart your browser. It sometimes helps to clear your cache as well.
If you do those things and you still have the problem, double check that you don't actually have an achievement outstanding.
If you still get the exclamation point, submit a ticket to support.
